<b>Introduction</b>This book entitled “Culturally tailored case studies for facilitators and carers in the iSupport for dementia program” with accompanied videos is an education and coaching resource to enable the implementation of the research project entitled “A ‘culturally tailored iSupport model’ for carers of people with dementia”. The culturally tailored iSupport model (Hereafter: CT-iSupport) is a multicomponent intervention for carers of people with dementia (PWD) from culturally and linguistically diverse (CALD) communities. The CT-iSupport comprises: 1) facilitator-enabled access to, and utilisation of care services to meet the care needs of PWD; 2) carer support groups for carers; 3) dementia care education for carers using the iSupport program in their preferred language; and 4) carers’ feedback on care services.<br>This project includes four aims. These are to:· Aim 1: Co-design strategies and co-create resources to implement the iSupport model.· Aim 2: Determine the intervention effectiveness.· Aim 3: Determine implementation strategies.· Aim 4: Determine the cost-effectiveness of the intervention.<br>The unfolding case studies with accompanied videos presented in this book address aim 1. These resources were co-designed and co-produced with stakeholders to complement the World Health Organization’s (WHO) iSupport for dementia program in an Australian socio-cultural context. The evidence supporting the unfolding case studies comes from our focused literature review that reported real cases; submitted cases from stakeholders; and consultations with stakeholders. These unfolding case studies emphasise challenges carers encounter at the time of dementia diagnosis, throughout everyday dementia care, and during transition moments.<br><b>Outline of unfolding case studies in each iSupport module</b>In total, we have developed 20 unfolding case studies which include 56 related case studies. We have organised these unfolding case studies to relate the WHO iSupport program modules as showed in Table 1. Each unfolding case study focuses on dementia caregiving in a CALD community. However, stakeholders across different CALD communities have contributed and made a collective effort in the development of each unfolding case study. We believe the collaboration, collective action and reflection on dementia caregiving across CALD communities will generate synergy. Sharing each other’s strengths and care services will enhance education, coaching and support for carers.<br><b>The structure of the unfolding case study</b>Each unfolding case study is organised as follows:Brief introduction to the unfolding case studyActorsThe links to the case study videosCase study scriptso Challenges and stresses the carer experienceso Carer’s learning journey that enables them to cope with the challengeso The care outcomes the carer has achievedCoaching tools for carers to achieve their care goalsCoaching tools for facilitators to enable carers to achieve their care goals<br><b>How to use the unfolding case studies</b>The unfolding case study can be used to complement the WHO iSupport program or as a standalone education and coaching resource based on the carers’ dementia learning needs. We have developed a “Carer learning needs assessment and action taking tool” (Appendix 1) to enable carers to assess their learning needs, and select relevant unfolding case studies to meet their learning needs. We encourage carers to watch the unfolding case study videos first and then read the unfolding case study when they need to do so. We also encourage carers to develop their own coaching plan after they have watched and/or read the unfolding case study to achieve their goals in the care of their loved ones.
<b>引言</b>本配套视频资源《面向痴呆症iSupport项目引导者与照护者的定制化文化案例集》,是用于推进题为《面向痴呆症患者照护者的定制化文化iSupport模式》研究项目落地的教育与辅导资源。本定制化文化iSupport模式(以下简称CT-iSupport)是面向来自文化和语言多元(Culturally and Linguistically Diverse,CALD)社区的痴呆症患者(People With Dementia,PWD)照护者的多组分干预方案。CT-iSupport包含四大组成部分:1)引导者协助照护者获取并利用照护服务,以满足痴呆症患者的照护需求;2)照护者互助支持小组;3)采用照护者偏好语言的iSupport项目内容,面向照护者开展痴呆症照护教育;4)照护者对照护服务的反馈机制。<br>本项目包含四大研究目标,具体如下:· 目标1:联合设计落地策略、共创配套资源,以推进iSupport模式的实施;· 目标2:评估干预方案的有效性;· 目标3:明确干预实施策略;· 目标4:评估干预方案的成本效益。<br>本手册中配套视频的动态案例集正是针对目标1所开发的内容。本系列资源由多方利益相关者联合设计与共创,旨在澳大利亚社会文化语境下补充世界卫生组织(World Health Organization,WHO)的痴呆症iSupport项目。支撑本动态案例集的实证依据包括:收录真实案例的定向文献综述、利益相关者提交的实际案例,以及与利益相关者开展的咨询研讨。本系列动态案例聚焦照护者在痴呆症确诊时、日常照护过程中以及转衔阶段所面临的各类挑战。<br><b>各iSupport模块下的动态案例集概述</b>本次研究共开发20组动态案例集,涵盖56个相关子案例。我们将本系列动态案例与表1所示的WHO痴呆症iSupport项目模块进行了对应匹配。每组动态案例均聚焦于CALD社区内的痴呆症照护场景,且所有案例均由来自不同CALD社区的利益相关者共同参与开发。我们相信,跨CALD社区的痴呆症照护协作、集体行动与反思将产生协同效应;通过共享彼此的优势资源与照护服务,能够进一步提升对痴呆症照护者的教育、辅导与支持力度。<br><b>动态案例集的结构</b>每组动态案例的结构如下:1. 动态案例简介;2. 参与角色;3. 案例视频链接;4. 案例脚本: - 照护者经历的挑战与压力; - 照护者应对挑战的学习历程; - 照护者达成的照护成果;5. 帮助照护者达成照护目标的辅导工具;6. 帮助引导者协助照护者达成照护目标的辅导工具。<br><b>动态案例集的使用方法</b>本动态案例集既可作为WHO痴呆症iSupport项目的补充资源,也可根据痴呆症照护者的学习需求,作为独立的教育与辅导资源使用。我们开发了"Carer learning needs assessment and action taking tool"(附录1),帮助照护者评估自身学习需求,并选择匹配的动态案例集以满足学习目标。我们建议照护者可优先观看案例视频,若有需要再阅读配套案例文本。同时,我们鼓励照护者在观看或阅读完案例内容后,结合自身照护所爱之人的目标,制定专属的辅导计划。
