This dataset is an Application Demonstration Report on the Application of Internet + Elderly Care Tight-knit Medical Consortium. The Tight-knit Medical Consortium Demonstration Base targets the elderly population with chronic disease and urgent care needs, carries out its application relying on the "Internet + Hospital" cloud platform, and has explored and condensed the "1233 Precision Transitional Care" model. Within the demonstration area, the effective care rate for the elderly increased by 15.5%, the training rate of primary care workers increased by 32%, which improved the quality, efficiency, and continuity of medical and health services and better met the health care needs of elderly patients with chronic diseases.
