The contents of these files varies. All files contain the official departmental admission form. Details on the face of the card are the same as NRS 4968 Admission cards - Personal and family history . On the reverse is recorded, date of admission, ailment, capacity to work, medical officer's signature, date of and reason for discharge. Particulars of deceased patient form records: name, age, card number, admission number, ward, date of admission, date and time of death, name, relationship, address, time and date of notification of relatives or friends, particulars of funeral, including date of burial, cemetery, section and grave number, religion, estate particulars, property in store and disposal and general remarks.
