*Significant at p†Model 1 for all-cause mortality (N = 14029) and for CVD-related mortality (N = 136981) adjusted for age, gender, race/ethnicity, country of origin and education level and reduced by 124 subjects due to missing data on country of origin and/or education level.‡Model 2 for all-cause mortality (N = 13611) and for CVD-related mortality (N = 13569) adjusted for age, gender, race/ethnicity, country of origin, education level, body mass index (kg/m2), smoking status, diabetes status and non-steroidal anti-inflammatory drug use. Model 2 for all-cause mortality reduced by an additional 418 subjects and model 2 for CVD-related mortality reduced by an additional 322 subjects due to missing data on body mass index, smoking status, diabetes status and/or non-steroidal anti-inflammatory drugs.
