aMultiple imputation was used to impute missing values of baseline CD4 count and clinical stage, and to impute outcomes in patients lost to follow-up (see Table 1).Abbreviations: NNRTI, non-nucleoside reverse transcriptase inhibitor; PI, protease inhibitor.
