Extended Cohort for E-health, Environment and DNA (EXCEED): Exceed Self Completion Questionnaire

All EXCEED participants were invited to take this questionnaire. It covered topics about some core demographic information (such as gender), personal habits on smoking and drinking and some basic information on their registered GP.
