<b>Obesity Indices, Inflammation, and Sex Steroid Hormones in Children and Adolescents: A Cross-Sectional Analysis of NHANES 2013-2016</b>

<b>Background</b>: Compared with the traditional body mass index (BMI), newer obesity indices such as the body roundness index (BRI) and abdominal body shape index (ABSI) have exhibited superior efficacy in characterizing body fat distribution patterns. However, the associations between BRI, ABSI and sex steroid hormones in children and adolescents remain clear. Furthermore, although systemic inflammation is recognized to disrupt hormonal regulation, the combined effects of obesity indices and the overall systemic inflammation index (AISI) on sex steroid hormones have not been fully investigated. <b>Methods</b>: This cross-sectional analysis utilized data from 3,326 participants aged 6 to 19 years enrolled in the National Health and Nutrition Examination Survey (NHANES, 2013–2016). Weighted linear regression models were employed to evaluate the individual and combined impacts of obesity indices and AISI on sex steroid hormones, specifically estradiol (E2), total testosterone (TT), sex hormone-binding globulin (SHBG) and free androgen index (FAI). <b>Results</b>: Among the 3,326 participants, the mean age was 12.80 years, with 1,691 males. Compared with the reference group with low obesity indices and low AISI, the combination of high BMI and high AISI demonstrated the strongest associations with E2, SHBG and FAI, while the combination of high ABSI and high AISI exhibited the strongest association with TT. <b>Conclusions</b>: BMI, BRI, ABSI and AISI are significantly associated with sex steroid hormones in children and adolescents. Importantly, the combined assessment of obesity indices and inflammatory markers significantly enhanced the capacity to predict variations in sex steroid hormones.
