Sjogren’s syndrome (SS) is a long-term rheumatic autoimmune disease that affects multiple organs. It is not known whether ocular changes are relevant to the diagnosis of SS. This was addressed in the present study by examining microvascular changes in the retina of SS patients by optical coherence tomography angiography (OCTA).Individuals with SS and healthy controls were enrolled (n=12 per group). An en-face OCTA scan was performed on each eye. Images were segmented into 9 subregions and macular retinal thickness (RT) and superficial vessel density (SVD) were measured and compared between the 2 groups.Visual acuity (VA) differed significantly between SS patients (24 eyes) and controls (24 eyes) (P<0.001). In SS patients, inner RT was reduced in the inner superior region, outer RT was reduced in the outer nasal (ON) region, and full RT was reduced in the ON region compared to the control group (P<0.05). RT was negatively correlated with serum IgG level in the outer and full retina at ON regions (P < 0.05). SVD in the inner nasal, ON, and inner temporal regions was significantly lower in SS patients than in control subjects (P<0.05). SVD was positively correlated with full RT in the ON region in SS patients (P<0.05). The areas under the receiver operating characteristic curves for the diagnostic sensitivity of outer RT and full RT in the ON region for SS were 0.828 (95% confidence interval [CI]: 0.709–0.947) and 0.839 (95% CI: 0.715–0.963), respectively.The study was conducted in accordance with the Helsinki Declaration and was approved by the hospital’s ethics council. Before signing the informed consent statement, all participants fully understood the research methods and objectives.<br><br>
干燥综合征(Sjogren’s syndrome, SS)是一种累及多器官的慢性风湿性自身免疫性疾病。目前尚不明确眼部改变是否与干燥综合征的诊断相关。本研究通过光学相干断层扫描血管成像(optical coherence tomography angiography, OCTA)观察干燥综合征患者视网膜微血管改变,以此探讨该问题。本研究纳入干燥综合征患者与健康对照各12例(每组n=12),对每只眼均实施面扫式光学相干断层扫描血管成像(en-face OCTA)检查。将扫描图像分割为9个亚区,测量黄斑区视网膜厚度(retinal thickness, RT)与表层血管密度(superficial vessel density, SVD),并对两组受试者的上述指标进行比较。
干燥综合征患者(24只眼)与健康对照(24只眼)的视力(visual acuity, VA)存在显著差异(P<0.001)。与对照组相比,干燥综合征患者的内上象限视网膜内层厚度降低,外鼻象限(outer nasal, ON)视网膜外层厚度及全层视网膜厚度均出现降低(P<0.05)。在外鼻象限的视网膜外层及全层,视网膜厚度与血清IgG水平呈负相关(P<0.05)。干燥综合征患者内鼻象限、外鼻象限及内颞象限的表层血管密度显著低于健康对照(P<0.05)。干燥综合征患者外鼻象限的表层血管密度与全层视网膜厚度呈正相关(P<0.05)。外鼻象限视网膜外层厚度及全层视网膜厚度用于诊断干燥综合征的受试者工作特征曲线(receiver operating characteristic curve, ROC)下面积分别为0.828(95%置信区间[CI]:0.709~0.947)与0.839(95%置信区间[CI]:0.715~0.963)。本研究遵循《赫尔辛基宣言》开展,并获得医院伦理委员会批准。所有受试者在签署知情同意书(informed consent statement)前,均已充分了解本研究的方法与目的。
