VPRS 7447 Admission Cards of Male and Female Patients

This series records the admission details of each patient admitted to Yarra Bend Asylum between 1912 and 1925. It was maintained concurrently with the Registers of Patients VPRS 7417.The front of each card records the admission details of each patient:nameagesexmarital statusoccupationdate of receptionwhether previously at a receiving houseunder what section of the Lunacy Act committedcause of insanitynames of medical practitioners certifyingprevious place of abodenames and addresses of relatives.The back of each card records the diagnosis, prognosis, and mental and physical condition (signed by medical certificate) on admission. There is also room for discharge details (boarded out, transfer, probation leave, death) on back of card but this is rarely completed.

这个系列记录了1912年至1925年间入院雅拉本德精神病院（Yarra Bend Asylum）的每位患者的入院详情，与患者登记册VPRS 7417同步维护。每张卡片正面记录患者的入院信息，包括：姓名、年龄、性别、婚姻状况、职业、收治日期、是否曾入住接收所、依据《精神错乱法案》（Lunacy Act）哪一条款入院、精神错乱原因、出具证明的执业医师姓名、既往居住地、亲属姓名及地址。卡片背面记录入院时的诊断、预后及身心状况（由医疗证明签署），还留有出院详情填写栏（含寄养、转院、试假、死亡），但该部分极少填写。
