Aims: The main aim of the thesis was to evaluate and compare the effectiveness of two therapeutic approaches using robot-assisted glove Gloreha Sinfonia to perform Activity of daily living in patients after acquired brain injury. Methods: The study is an empirical quantitative research, a monocentric randomized controlled, simply blinded study. The study compares two groups, group A (n = 20) used a robot-assisted Gloreha glove with a bimanual approach, group B (n = 20) had therapy with a robot-assisted Gloreha glove with an unimanual approach. All patients were evaluated for eligibility and underwent initial testing (T1). Patients in both groups had therapy (unimanual / bimanual) for three weeks (15x), retest (T2) was performed after completion of the intervention, and a follow-up evaluation was performed after 1 month (T3). Patients were always evaluated by the Upper Extremity Motor Activity Log (UE MAL), Motor Assessment Scale (MAS), Action Research Arm Test (ARAT), Box and Block test (BBT) and Motricity Index (MI). Results: According to the results of the MAS test in group A, there was a statistically significant improvement in upper limb function in category 8 at the time T2 (p = 0.038) and at the time T3 (p = 0.044) and in category 7 at the time T3 (p = 0.015). UE MAL in group A showed a marginal statistically significant improvement in UE function at the time T2 (p = 0.07) and at the time T3 (p = 0.1). There was a statistically significant increase in muscle strength according to MI at the time T2 for the shoulder (p = 0.053) and at the time T3 for the elbow (p = 0.027), a marginal significant increase in muscle strength occurred at both times (p = 0.080 and p = 0.075). Conclusion: The inclusion of bimanual robot-assisted therapy appears to be effective in restoring functional utilization of the upper limb during activities of daily living in patients after acquired brain injury with moderate to severe hemiparesis. 研究目标:本研究的核心目标为评估并对比两种采用Gloreha Sinfonia型机器人辅助手套为获得性脑损伤患者开展日常生活活动(Activity of daily living)训练的治疗方案的有效性。 研究方法:本研究为实证定量研究,采用单中心随机对照单盲试验设计。研究设置两组对照:A组(n=20)采用双侧协同训练模式,使用Gloreha Sinfonia型机器人辅助手套进行治疗;B组(n=20)采用单侧训练模式,使用同款手套进行治疗。所有受试者均通过入组资格筛查,并完成初始测评(记为T1)。两组受试者均接受为期3周(共15次)的单侧/双侧协同训练治疗,干预结束后开展复测(记为T2),并于干预后1个月进行随访评估(记为T3)。所有受试者均采用以下测评工具进行评估:上肢运动活动日志(Upper Extremity Motor Activity Log, UE MAL)、运动评估量表(Motor Assessment Scale, MAS)、动作研究手臂测试(Action Research Arm Test, ARAT)、箱块测试(Box and Block Test, BBT)以及运动功能指数(Motricity Index, MI)。 研究结果:A组运动评估量表(MAS)结果显示,其上肢功能评分在第8项于T2时刻(p=0.038)及T3时刻(p=0.044)均出现统计学显著提升,在第7项于T3时刻(p=0.015)亦出现统计学显著提升。A组上肢运动活动日志(UE MAL)结果显示,其上肢功能在T2时刻(p=0.07)及T3时刻(p=0.1)呈现边缘统计学意义的改善。运动功能指数(MI)结果显示,A组肩部肌肉力量在T2时刻出现统计学显著提升(p=0.053),肘部肌肉力量在T3时刻出现统计学显著提升(p=0.027);两处肌群在两个时间点均呈现边缘显著的肌力提升(p=0.080与p=0.075)。 研究结论:对于伴有中重度偏瘫的获得性脑损伤患者,双侧协同式机器人辅助治疗可有效恢复其在日常生活活动中的上肢功能应用能力。双侧协同式机器人辅助治疗同时对患肢整体肌肉力量具有积极改善作用。
