Application Value of Intelligent Quick Magnetic Resonance (IQMR) Technology in Preoperative Evaluation of Primary Hemifacial Spasm

Objective To explore the application value of Intelligent Quick Magnetic Resonance (IQMR) technology in preoperative evaluation of primary hemifacial spasm (HFS) and provide accurate imaging evidence for microvascular decompression (MVD).Methods A total of 42 patients with unilateral HFS scheduled for MVD were enrolled. All patients underwent conventional MRI sequences and IQMR sequences (including 2D T1WI/T2WI, 3D-FIESTA, 3D-TOF) before surgery. The scanning time of the two sequences was compared. Image quality was evaluated by subjective scoring (5-point scale) and objective indicators (SNR, CNR). Taking intraoperative findings of MVD as the gold standard, the diagnostic efficacy was analyzed.Results The total scanning time of the IQMR group was 37.14% shorter than that of the conventional group (286 s vs 455 s), with the 3D-FIESTA sequence shortened by 48.89% and the 3D-TOF sequence by 32.84%. In terms of subjective scoring, the median score of the IQMR group was 4.00 (4.0, 5.0), which was significantly higher than that of the conventional group [3.00 (3.0, 4.0)] (P<0.05), with excellent inter-observer consistency (Kappa>0.8). Objective indicators showed that the SNR of facial nerve, CNR of facial nerve-cerebrospinal fluid (CSF), and CNR of artery-CSF in the 3D-FIESTA and 3D-TOF sequences of the IQMR group were significantly higher than those of the conventional group (all P<0.05). Both groups achieved 100% diagnostic sensitivity and accuracy, with consistent identification of responsible blood vessels in 41 cases between preoperative assessment and intraoperative findings.Conclusion IQMR technology can significantly shorten scanning time while improving image quality, accurately display the anatomical relationship between the facial nerve and responsible blood vessels with reliable diagnostic efficacy, and provide an efficient and high-quality imaging solution for preoperative evaluation of HFS.
