Supplemental materials for "Fostering Collaboration by Leading Communities of Practice"

Each interview protocol contains the questions that were used in semi-structured interviews with participants. The survey questions were created and disseminated in Qualtrics. The study was determined to be exempt from ongoing IRB review (HUM00141251).
