ABSTRACT : Dysparenting, referring to inappropriate parental attitudes,is a vulnerability factor for mental disorders during adolescence and a therapeutic leverage, yet clinicians lack reliable tools to assess it in daily clinical practice. Moreover, the effect of this dysparenting on the amount of psychiatric care remains unclear.The Family & Care study aims to develop the At-Risk Family Interaction Scale (ARFI), a comprehensive 30-item clinical scale, and to assess in a cross-sectional design, the impact of these at-risk family interactions on the care of adolescents(n=425)hospitalized in psychiatry and aged 13 to 19 years old.Factorial analysis shows that the ARFI scale consists of three main dimensions associated with Cohesion/Conflicts, Love/Hostility and Autonomy/Control with good psychometric properties.Multivariate regressions show that the ARFI Intensity score predicts the duration of hospital care, regardless of age, gender, medical severity on admission -assessed by the Global Assessment of Functioning scale-, the presence of maltreatment and psychiatric diagnoses. Moreover, the ARFI Diversity score(number of items present regardless of their severity) predicts both the number and duration of hospitalizations. At-risk family interactions are a determining dimension of psychiatric adolescent care, and the ARFI scale could constitute a valuable tool, not only for their holistic evaluation and treatment, but also for their prevention.
摘要:不良养育(Dysparenting)指不当的父母养育态度,是青少年期精神障碍的易感因素,同时也是治疗干预的潜在靶点,但临床医师在日常临床实践中缺乏可靠的评估工具。此外,此类不良养育行为对精神科诊疗服务量的影响仍不明确。“家庭与照护”(Family & Care)研究旨在开发风险家庭互动量表(At-Risk Family Interaction Scale,ARFI)——一款包含30个条目、具备全面评估能力的临床量表,并采用横断面研究设计,评估此类风险家庭互动对13至19岁、共计425名精神科住院青少年的诊疗产生的影响。因子分析结果显示,ARFI量表包含三大核心维度,分别为亲密/冲突、关爱/敌意与自主/控制,且具备良好的心理测量学性能。多元回归分析表明,ARFI强度得分可独立预测住院治疗时长,不受年龄、性别、入院时基于整体功能评定量表(Global Assessment of Functioning scale,GAF)评估的病情严重程度、虐待史及精神疾病诊断的影响。此外,ARFI多样性得分(即无论严重程度高低的达标条目总数)可同时预测住院次数与住院时长。风险家庭互动是影响青少年精神科诊疗的关键维度,而ARFI量表不仅可用于此类互动的整体性评估与干预,还可应用于相关预防工作,具备重要的临床应用价值。
