Background: Primary brainstem hemorrhage (PBH), a lethal subtype of spontaneous intracerebral hemorrhage (ICH) comprising 6–10% of cases, causes acute autonomic dysregulation via compression of medullary nuclei, yielding 47–80% mortality and 40% respiratory failure rates. Conventional supportive therapies fail to restore hierarchical autonomic homeostasis, highlighting the need for targeted, closed-loop bioelectronic interventions.Objective: We propose the Neuro-Cardio-Respiratory Stabilization Platform (NCR-SP), a rigorous conceptual framework grounded in the Hierarchical Autonomic Integration Model (HAIM), to enable proactive stabilization through multimodal neuromodulation.Methods: HAIM formalizes the autonomic nervous system as a hierarchical network disrupted by PBH-induced singularities, modeled via stochastic differential equations (SDEs) incorporating heart rate (HR), respiratory rate (RR), and intracranial pressure (ICP) dynamics, with vagus nerve stimulation (VNS) feedback (η = 0.2 for ICP attenuation). NCR-SP integrates high-fidelity sensing (ICP/EEG/ECG/imaging), adaptive aVNS/diaphragmatic pacing, and AI-driven prediction (LSTM-ConvLSTM, AUC >0.95) using hydrogel-thin-film interfaces (yield >95%, stability >18 months). Reproducible Monte Carlo simulations (n=500) validate efficacy.Results: Simulations demonstrate 62% collapse risk reduction (95% CI: 58.5–65.5%; from baseline 62% to 0%), HRV stabilization (SDNN 3.7 ms), and ICP mitigation via VNS, aligning with empirical VNS data (39% hematoma reduction; 49% UE-FM gains in stroke rehabilitation).Conclusions: NCR-SP redefines PBH management as a unified, evidence-based paradigm, with ICER <US$35,000/QALY, ethical safeguards, and translational roadmap (porcine/clinical Phase I/IIa). This framework extends VNS applications, heralding precision neurocritical care for global equity.
