the purpose of the data was to explore the performance of the bacterial filters (BF) on the incidence of ventilator-associated pneumonia.totally, 240 patients considered to require MV for more than 48h on admission were enrolled. 120 were assigned to BF group, 120 were assigned to NBF group; 29 patients (16 in BF group, 13 in NBF group) were secondarily excluded for the duration of intubation was less than 48h. one patient was excluded for the lack of chest radiograph before intubation. The characteristics of the remaining patients (104in BF group and 106 in NBF group) were analyzed at last.The characteristics of the two group were shoqed in table1.Univariable logistic analysis was applied to demonstrate the factors contributing to the development of VAP. The results were showed in the Table 2.The retrospective subgroup analysis results were showed in Table 3. cox proportional hazards regression model was showed in table4. in clusion, we found that bacterial filters placed on the inspiratory limbs of breathing circuit did not reduce VAP in ICU patients ventilated for more than 48h when the gas was humidified with heated water-bath humidifiers with double heater-wire circuit. Interestingly, the filter might protect the patients from occurrence of VAP when ventilated for not long enough.

本数据集旨在探讨细菌过滤器(bacterial filters, BF)对呼吸机相关性肺炎(ventilator-associated pneumonia, VAP)发生率的影响。研究最终纳入入院后需接受机械通气(mechanical ventilation, MV)时长超过48小时的患者共240例,将其分为BF组(120例)与非细菌过滤器组(NBF组,120例)。后续因插管时长不足48小时排除29例(BF组16例,NBF组13例),另因插管前缺失胸部X线片排除1例。最终纳入分析的患者为BF组104例、NBF组106例,两组患者的基线特征见表1。本研究采用单因素logistic回归分析,以筛选与VAP发生相关的影响因素,分析结果见表2。回顾性亚组分析结果见表3,Cox比例风险回归模型(Cox proportional hazards regression model)的分析结果见表4。综上,本研究发现:当采用带双加热丝回路的加热水浴式湿化器进行气道湿化时,在呼吸回路吸气端放置细菌过滤器,并未能降低ICU中机械通气时长超48小时患者的VAP发生率。值得注意的是,对于通气时长不足的患者,该过滤器或可对VAP的发生起到保护作用。
