We report a ceftriaxone and multidrug resistant Neisseria gonorrhoeae, GK 124, acquired by a heterosexual man in Denmark in 2017. Following treatment failure with empiric azithromycin for urethritis N. gonorrhoeae was cultured. It belonged to NG-MAST genogroup 1624 and was MLST-1903. Whole genome sequencing identified a number of resistance genes and novel alterations found in the penA gene leading to pan-resistance. Treatment was however successful with ceftriaxone 0.5g i.m. and azithromycin 2g p.o.
