2016 Community Health Worker Group Interaction Forms, Alabama and Louisiana

Group Interaction Forms (GIFs) are designed to collect data on group interactions by community health workers (CHW) in clinics or community-based organizations (CBOs) in Alabama and Louisiana. Community health workers will administer and submit forms quarterly to the University of South Alabama’s Coastal Resource and Resiliency Center (CRRC) beginning May through December 2016. CRRC will gather, input, analyze, and report data provided by the CHWs to evaluate the utility of these CHWs in building community resilience.

群体互动表单（Group Interaction Forms，GIFs）旨在收集阿拉巴马州与路易斯安那州境内诊所及社区组织（community-based organizations，CBOs）中社区卫生工作者（community health workers，CHW）开展群体互动的相关数据。社区卫生工作者需自2016年5月至12月期间，每季度向南阿拉巴马大学海岸资源与韧性中心（Coastal Resource and Resiliency Center，CRRC）提交填写完成的表单。该中心将对社区卫生工作者提交的数据进行汇总、录入、分析与报告，以评估此类社区卫生工作者在构建社区韧性过程中所发挥的效用。
