The association between lifestyle and multimorbidity conditions of patients with gastric diseases among the middle and elderly population in China:Based on CHARLS 2015 database analysis

Objective This study aimed to explore the association between the lifestyle and multimorbidity conditions of patients with gastric diseases among the middle and elderly population in China and to provide evidences for developing a comprehensive intervention program for gastric diseases of the middle and elderly population in China.Methods A cross-sectional study was conducted in this study, the data from the China Health and Retirement Longitudinal Study(CHARLS) 2015 database with complete interesting variables in the middle and elderly people aged ≥45 years were included in this study. The outcome measures were multimorbidity conditions of gastric diseases, and the interesting factors were lifestyle variables including smoking, alcohol consumption, physical activity and BMI. And general characteristics such as gender and age as well as physical examination information were the adjusted variables. The multimorbidity conditions of patients with gastric diseases were described , and multivariate logistic regression analysis was performed to explore the association between the lifestyle and the various different multimorbidity conditions of patients with gastric diseases.Results A total of 11749 subjects included in the final analysis and 2748 participants had gastric diseases with the prevalence rate of 23.4% (95% CI:22.6%-24.2%) among peoples aged ≥45 years in China.A total of 2,151 patients with gastric diseases had multimorbidity conditions and the prevalence of multimorbidity was 78.3%(95% CI:75.0%-81.7%). The top three binary comorbidity rates of gastric diseases with other chronic diseases were gastric diseases+arthritis (12.9%,95% CI:12.2%-13.5%),gastric disease+hypertension (6.1%,95% CI:5.7%-6.6%), gastric disease+heart disease (4.6%,95% CI:4.2%-5.0%),respectively.The top three ternary multimorbidity rates were gastric disease+arthritis+hypertension(3.7%,95% CI:3.4%-4.1%),gastric disease+arthritis+heart disease (3.1%,95% CI:2.8%-3.4%) and gastric disease+arthritis+pulmonary disease (2.7%, 95% CI:2.4%-3.0%),respectively. After adjusting for the other variables,the independent determinants of comorbidities of the patients with gastric diseases were alcohol drinking >1 time per month,overweight and obesity. Moderate physical activity, maransis, overweight, and obesity were associated with comorbidity of gastric diseases+arthritis, and with relatively stable outcomes in male. Mild physical activity, moderate physical activity, heavy physical activity, maransis, overweight, and obesity were associated with multimorbidity of gastric diseases+arthritis+hypertension, respectively.Conclusions Gastric diseases is one of the most common chronic diseases and multimorbidity diseases in in the middle-aged and elderly people in China. The most binary multimorbidity is comorbidity of gastric diseases with arthritis, and the most ternary multimorbidity is multimorbidity of gastric diseases with arthritis and hypertension. Physical inactivity and too large or too small BMI are independent determinants of the different comorbid conditions of stomach diseases among peoples aged ≥45 years in China.
