The essay takes into consideration the health policies and practices implemented in Terra d'Otranto between 1861 and 1949 in the wake of sporadic and epidemic outbreaks of smallpox, as documented in the State Archives of Lecce. The medical reports of local physicians and health officers along with correspondences between central and local authorities, detail the magnitude of the disease in terms of infections and mortalities and highlight the critical issues that emerged in the implementation of the measures to fight against the outbreaks.
