Menopause, despite being the most common endocrine deficiency state, has been traditionally framed as a normal life event rather than a treatable hormonal deficiency with proven efficacy in disease prevention. Current menopause treatment guidelines implement a palliative, symptomatic care model, resulting in missed opportunities for comprehensive health promotion and disease prevention in post-reproductive age women. While the reproductive functions or estrogen and progesterone are well known, their systemic roles are underappreciated in medicine, in part because no specialty focuses holistically on their impact beyond fertility. The absence of an integrated clinical framework of gynecologic endocrinology has left millions of women underserved in managing the multisystem effects of hormonal deficiency. This article details the critical functions of estradiol and progesterone, the consequences of their loss, and the conceptual framework needed to guide integrated menopause care. Gynecologic endocrinology, as proposed here, is a clinical discipline focused on the integrated, systemic management of estradiol and progesterone deficiency beginning at menopause. It is distinct from reproductive endocrinology and infertility, which addresses hormonal care during the reproductive years.
