Background: The study aimed to increase access to and uptake of COVID-19 testing and vaccination among people who inject drugs (PWID), a population highly vulnerable to COVID-19 infections and vaccine hesitancy. Materials/Methods: The study enrolled participants from San Diego and Tijuana and conducted interviews that helped shape the intervention offered at a mobile syringe services program (SSP) developed with guidance from the Community and Scientific Advisory Board. Peer counselors provided rapid antigen testing, confirmatory PCR, and referrals to nearby vaccination centers until the state began offering vaccines on-site. Vaccine uptake was independently evaluated through ongoing study visits, which included self-reported interview data, electronic health record linkage, and antibody analysis to distinguish natural infection from vaccine-induced immunity. Outcome/Impact: The study generated estimates of effect sizes for a future efficacy trial and shared findings with the RADx-UP consortium, policymakers, and program planners. By demonstrating how SSPs could serve as effective touchpoints for testing and vaccination, the study informed broader efforts to leverage SSPs nationwide to reach marginalized PWID and strengthen pandemic preparedness infrastructure to reduce COVID-19 health disparities.
