This dataset was collected through on-site questionnaire surveys by investigators (staff from community health service centers or village doctors) in the survey areas of Hunan, Hubei, Anhui, Jiangxi, and Henan Provinces from 2020 to 2022. It includes the following fields: province, prefecture-level city/prefecture, district/county, questionnaire type, questionnaire completion year, study subject ID, gender, age (years), ethnicity, educational level, awareness of hypertension, lipid measurement status, blood glucose measurement status, awareness of atrial fibrillation, awareness of coronary heart disease, regular blood pressure measurement status, blood pressure control range, lipid control range, adherence to antihypertensive drugs, adherence to lipid-regulating drugs, adherence to hypoglycemic drugs, smoking cessation education, alcohol restriction education, weight control education, low-sodium diet education, low-fat diet education, diabetes health education, stroke recognition education, stroke medication guidance, health education channels - medical staff, health education channels - community, health education channels - television, health education channels - introductions from others, health education channels - online sources, health education channels - others.
