Treatment with injury (avoidable), share (%)

The proportion of care sessions where the patient has received one or more injuries and where the injury was assessed as avoidable (categories "avoidable" or "probably avoidable" in the journal review). Care injury refers to injury to a patient that could have been avoided if appropriate measures had been taken during the patient's contact with the healthcare system. Examples of healthcare injuries are pressure ulcers or healthcare-associated infections acquired during hospitalisation. The review team, consisting of doctors and nurses, makes a random selection of a number of medical records per month. Thereafter, medical records from these care sessions are reviewed to seek markers, i.e. medical records that involve an increased risk of an injury having occurred. If the patient has suffered an injury during the treatment, the review team makes an assessment of the type of injury, how serious it is and whether it could have been avoided. Data are available according to gender breakdown.本数据集统计的是患者在诊疗过程中发生1次及以上医疗伤害（Care Injury），且该伤害经评估为可避免（在期刊综述中分类为"可避免"或"大概率可避免"）的诊疗场次占比。医疗伤害指患者在接触医疗系统过程中，若采取恰当措施即可避免的患者损伤。医疗伤害示例包括压疮或住院期间获得的医疗相关性感染。由医师与护士组成的评审团队每月会随机抽取若干份医疗记录。随后对上述诊疗场次的医疗记录进行评审，以查找风险标记——即存在更高概率发生医疗伤害的医疗记录。若患者在诊疗期间发生伤害，评审团队将对伤害类型、严重程度及是否可避免进行评估。数据集可按性别分类提供数据。
