Questionnaire for AI Attitudes, AI Anxiety, and Technology Acceptance among Healthcare Decision-Makers in Jordan

This record contains the Arabic and English versions of the questionnaire used in the study titled “The Role of Technology Acceptance, AI Anxiety, and Demographic Factors in Jordanian Healthcare Decision-Makers’ Attitudes Toward Artificial Intelligence.” The questionnaire was designed to assess attitudes toward artificial intelligence, technology acceptance, AI-related anxiety, and demographic/professional characteristics among healthcare decision-makers in Jordan. The instrument includes demographic questions, the General Attitudes toward Artificial Intelligence Scale (GAAIS), Technology Acceptance Model (TAM) items, and Artificial Intelligence Anxiety Scale (AIAS) items. The questionnaire was administered in both Arabic and English. The Arabic version was developed using a forward–backward translation process, reviewed by bilingual experts for clarity, cultural relevance, and conceptual equivalence, and pilot tested with healthcare professionals before use in the study.
