<p>This is the result data of a case study on system failures in construction and spaceflight projects, conducted to learn lessons for designing and building safe extraterrestrial systems. Ensuring that the case set included diverse cases and covered all specific technical challenges of extraterrestrial construction, the selection resulted in 40 cases comprising 10 space systems, 13 buildings, 10 bridges and tunnels, and 7 systems in specialized environments.</p> <p>The failure cases were modeled using the Framed-and-Layered Accident Pathogen Propagation (FLAPP) model—an accident model specifically developed for construction system failures—and graphically illustrated as FLAPP Matrix representations. To discover trends, differences, and commonalities in the underlying mechanisms of system failures, instances of pathogen threads, design-induced strains, and inadequate organizational factors—concepts explaining the incubation process of design decisions into system failures—were identified for each failure case.</p>
