2017 Community Health Worker Group Interaction Forms, Alabama and Louisiana

Group Interaction Forms (GIFs) were designed to collect data on group interactions by community health workers (CHW) in clinics or community-based organizations in Alabama and Louisiana. Community health workers administered and submitted forms quarterly to the University of South Alabama’s Coastal Resource and Resiliency Center (CRRC) beginning January through September 2017. CRRC will gather, input, analyze, and report data provided by the CHWs to evaluate the utility of these CHWs in building community resilience.
