该数据集为杭州市第一人民医院在 2020–2025 年医养结合管理与智慧评估活动中形成的数据集。数据涵盖老年患者的人口学信息（年龄）、基础疾病及共病情况（糖尿病、高血压、肺部疾病、心血管疾病、脑血管疾病、阿尔茨海默病、帕金森、运动神经元病、肾功能不全等）、临床干预信息（机械通气、气管插管/气管切开、中心静脉置管、负压封闭引流术、手术治疗、≥3 种抗生素使用情况）、护理及并发症信息（压疮分期、血流感染发生情况）、实验室指标（白细胞、血小板、血红蛋白、肝肾功能指标、电解质、血脂、血清白蛋白、C-反应蛋白、降钙素原）及随访结果（30 天死亡、出院/死亡日期）等多维度信息。数据集经过脱敏和标准化处理，可作为人工智能算法与模型训练的重要数据基础，支持老年患者健康风险评估、并发症预测、护理干预优化及医养结合个性化管理，同时可服务于智慧医疗、科研教学和真实世界研究，推动医养结合精细化管理和智能化决策。
This dataset was developed by Hangzhou First People's Hospital during the integrated medical and elderly care management and smart assessment activities spanning 2020 to 2025. It contains multi-dimensional data of geriatric patients, including demographic information (age), underlying diseases and comorbidities (diabetes, hypertension, pulmonary diseases, cardiovascular diseases, cerebrovascular diseases, Alzheimer's disease, Parkinson's disease, motor neuron disease, renal insufficiency, etc.), clinical intervention information (mechanical ventilation, tracheal intubation/tracheotomy, central venous catheterization, negative pressure wound therapy (NPWT), surgical treatment, use of ≥3 types of antibiotics), nursing and complication information (stages of pressure injuries, incidence of bloodstream infections), laboratory indicators (white blood cell count, platelet count, hemoglobin, hepatic and renal function indicators, electrolytes, blood lipid profile, serum albumin, C-reactive protein, procalcitonin), and follow-up outcomes (30-day mortality, discharge or death dates). The dataset has been de-identified and standardized, serving as a critical data foundation for training artificial intelligence algorithms and models. It supports health risk assessment, complication prediction, nursing intervention optimization and personalized integrated medical and elderly care management for geriatric patients, and also caters to smart healthcare, scientific research and teaching, as well as real-world research, promoting refined management and intelligent decision-making for integrated medical and elderly care services.
