We are building a computational system that cross-references collateral sensitivity data from 8 published laboratory studies against clinical MIC surveillance data. Our preliminary analysis of 90,000 BV-BRC antibiograms revealed that only 3% of lab-derived collateral sensitivity predictions validate clinically at the population level, primarily due to plasmid-mediated co-resistance. ATLAS MIC data would allow us to (1) validate collateral sensitivity pairs with quantitative MIC values rather than binary S/R categories, (2) extend validation to Klebsiella pneumoniae, S. aureus, A. baumannii, and E. faecium, and (3) perform temporal trend analysis of collateral sensitivity patterns across 2020-2024. This research aims to improve antibiotic stewardship by identifying which empirically-validated collateral sensitivity pairs can inform clinical antibiotic selection.
