Open cost-consequence model (code, parameter tables with provenance tags, and results JSON) supporting the manuscript "Cost-consequence model of closed-loop patient-safety learning in Sweden, with an eleven-country comparison and a distributional (equity) analysis." Health-system perspective, SEK 2024, annual plus discounted 5-year, comparing the status-quo Swedish Lex Maria / Händelseanalys / NITHA apparatus against an action-coupled (closed-loop) reform. Contains the Sweden model, the harmonised eleven-country comparison with per-country cost-audit memos and adversarial verification reports, and the distributional (equity) analysis — sufficient to reproduce every reported number, and intended for independent computational reproduction (CODECHECK). Effectiveness inputs are borrowed from the external literature, not measured by SPIRA. Code is licensed AGPL-3.0-only; data and documentation under CC BY 4.0.
