Social Determinants of Health

**Description**
Experian social determinants of health (SDOH) audiences can provide insight into transportation, financial and technology barriers to care, medication, housing, and food to improve your population's health, care coordination, and risk stratification efforts and offer an engagement strategy based on public health expertise.

**Social Determinants of Health Data:**

- Experian SDOH Data may not be used as exclusion criteria
- The Presence of Vehicle attributes, inferring lack of vehicle/transportation, may not be used in digital marketing use cases, nor for Automotive marketing of any kind
- Upon reasonable notice to Client, Experian may withdraw or decline to provide Client any Experian Data or Data Services if Client’s use of Data Services or Experian Data is the subject of a substantial, adverse, or documented consumer or patient reaction related to consumer or patient privacy issues
- As modification to the Permitted Use(s) set forth in this Order, Client’s Permitted Use of the SDOH Data is limited to (i) offering customized engagement strategies to (A) increase patient engagement and patient satisfaction and (B) drive targeted community healthcare programs tailored to patients with specific needs related to the specific factors included in the Data Services; and (ii) increasing care plan compliance by patients by offering a customized engagement strategy to each such patient.

**Use Cases**
These audiences help you connect patients more effectively to community resources and improve care quality.

- Data-based strategy for building a network of community partnerships/referrals that address gaps in care (food, housing, medication, care). Understand the SDOH needs of their patients and partner with community programs that meet those needs.
- Inform strategic focus for population health programming – For example, for those with high technology sophistication and medium to high Access to Care risk (far from a provider and no vehicle), prioritize for telehealth option, replacing the need for technology-based survey administration.
