Triglycerides are a type of fat found in the blood. When triglyceride levels become too high, this can increase the risk of heart and blood vessel disease, known as atherosclerotic cardiovascular disease (ASCVD), and can also lead to pancreatitis, a painful and sometimes dangerous inflammation of the pancreas. Millions of people worldwide have high triglycerides, especially those living with conditions such as type 2 diabetes (high blood sugar), obesity, or metabolic syndrome (a group of health problems that include high blood pressure, extra body weight, and abnormal cholesterol levels). Current treatments that lower triglycerides often do not reduce them enough to fully lower these risks. Because of this, several new medicines are being developed that target triglycerides more directly.
A different type of medicine, called a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, may offer even broader benefits. Tirzepatide, a member of this newer class of medicines, has been shown to lower body weight, improve blood sugar levels in people with type 2 diabetes, and reduce major heart-related events. Tirzepatide works by stimulating certain hormone pathways that help people feel fuller and improves how the body handles blood sugar, promoting weight loss. Because tirzepatide affects several related risk factors at the same time, it may help patients not only by lowering triglycerides but also by improving overall health more than treatments that only target blood fats.
In this project, we will study people with moderate or severe hypertriglyceridemia (high triglycerides) who would typically be considered for trials of new triglyceride-lowering drugs. We will examine how much tirzepatide lowers triglycerides in this group, which patient characteristics are linked to greater improvement, and how many people move from the moderate or severe range to healthier levels after treatment.
We will use statistical methods to re-analyze data already collected in previous clinical trials to compare tirzepatide with control groups (where participants do not receive the new treatment, but instead, they may receive a placebo (an inactive substance) or the current standard treatment) and to understand how factors such as weight, type 2 diabetes status, and starting triglyceride levels influence treatment results. Our findings may help doctors understand when GIP/GLP-1 receptor agonist treatments like tirzepatide could be considered as the initial treatment for people with high triglycerides and additional health risks.
