HIV Open Data Project: Office on Women's Health Progress Report Form

The Office on Women's Health (OWH) Progress Report Form is used to collect quarterly information from OWH-funded grantees. The goal of the report is to obtain information about the following topics: - Workplan progress - Implementation of intervention programs - Staff development and training - Linkages with other community organizations - Demographic characteristics of individuals enrolled in interventions - HIV status of individuals enrolled in interventions - Linkages to care and targeted intervention enrollment - Outreach activities and policy implementation - Program evaluation
