ACO Realizing Equity and Community Health Eligible Beneficiaries

Accountable Care Organization (ACO) REACH Eligible Beneficiary Public Use File (PUF) data details Medicare Beneficiaries who were used as the reference population for comparison to aligned to the ACO REACH Model, formerly Global and Professional Direct Contracting (GPDC) Model, including average risk scores and eligibility months. This data is redacted and does not include identifiable information. This dataset is based on the defined service areas of the ACOs in the ACO REACH Model. This dataset serves as a comparison set of all beneficiaries which are eligible for ACO REACH.<br><br>Includes annual data for 2021, 2022, and 2023.
