Colonization and infection of the same strain of KPC-producing Klebsiella pneumoniae ST11 was identified in two patients from our Transplant Unit. Loss and acquisition of antibiotic resistance genes was observed, revealing nosocomial plasticity of K. pneumoniae ST11 in our institution. On the one hand, advantage of surveillance cultures in readmitted patients for accurate antibiotic stewardships programs is documented, while Whole Genome Sequencing contributed to the identification of strains at a molecular level.
