Ulcerative colitis (UC) is a long-term condition where the body's immune system mistakenly attacks the colon (large intestine), causing ongoing inflammation. This leads to symptoms such as bloody diarrhea, frequent and urgent bowel movements, and a constant feeling of needing to pass stools, even though your bowels are already empty. UC affects over 0.5% of people in Western countries, and the number of cases is rising quickly in Asia and Latin America.
In the past 15 years, several new treatments have become available for people with moderate to severe UC. One of the newest types of medications are Janus kinase inhibitors, or JAK inhibitors. These are small molecules taken as pills that block specific parts of the immune system to reduce inflammation. Filgotinib is a JAK inhibitor that has helped around 37% of people with moderate to severe ulcerative colitis achieve long-term remission, which means their symptoms became minimal or disappeared. However, some patients still had poor outcomes during JAK inhibitor therapy.
Because JAK inhibitors do not work for everyone, researchers are trying to find early signs—called predictors—that can help show whether the treatment is likely to be successful. Predictors could include things like levels of certain proteins in stool (such as fecal calprotectin (FC)), symptoms reported by patients (like stool frequency or bleeding), and markers in the blood (like C-reactive protein (CRP)). However, the role of these preditors in JAK inhibitor-related therapies remains unclear.
This research will look at how well these early signs can predict long-term results for people taking JAK inhibitors for ulcerative colitis. It will use data from a large clinical trial called SELECTION which tested Filgotinib. Researchers will analyze the information to see if these common predictors can help identify who is most likely to benefit from JAK inhibitor treatment. The results of this study could help doctors and patients choose the most effective treatment more quickly and develop more personalized care plans.
