The municipality has a written and management-level routine for coordination in individual cases between LSS and elderly care within the social services. The routine was also followed up over the past year. Management level refers to the level within the organisation that has the mandate to decide on common procedures relating to several sub-activities/target groups. Routine refers to a specific approach to how activities are to be carried out around individuals who have complex needs and indicate the division of responsibilities when the activities are divided into different sub-activities/target groups. The procedure should be known and used by the responsible administrators. Coordination in individual cases refers to the coordination of resources and efforts between different areas of social services in order to achieve higher quality and greater efficiency. ‘Follow-up’ means that the actors concerned have systematically verified that the internal coordination procedure is being used and functioning as intended and that it has been revised if necessary.
