Questionnaire and Excel Sheet for Diagnostic Support in Vertigo Disorders

This package includes a dedicated questionnaire and an Excel-based evaluation sheet developed to assess multiple diagnostic criteria for vertigo disorders. These criteria have been established by both domestic and international medical associations. Information gathered from the questionnaire (available in a Word file) is input into the evaluation sheet (available in an Excel file), alongside examination findings, to determine conformity with various diagnostic criteria. The diagnostic criteria encompass diseases such as Meniere's disease, delayed endolymphatic hydrops, vestibular neuritis, and benign paroxysmal positional vertigo, as established by the 2017 Japan Society for Equilibrium Research (JSER) revision. They also include Meniere's disease, vestibular migraine, vestibular paroxysmia, persistent postural-perceptual dizziness, bilateral vestibulopathy, presbyvestibulopathy, and hemodynamic orthostatic dizziness/vertigo, as established by the Barany Society.
