We show that an intestinal-selective FXR agonist (FexD) is effective as a treatment for both type 1 and type 2 diabetes. In a type 2 diabetic mouse model (ob/ob), we find that Fex restores functional pancreatic β-cell mass and insulin secretion function by reducing intestinal inflammation and activating endogenous GLP1 (Glucagon-like peptide-1)-GLP1R (Glucagon-like peptide-1 receptor) cycling. Using an organoid model we show that this effect is mediated in part by suppression of TNFa-mediated intestinal inflammation.
