This is a randomized controlled study comparing DryNeedling to magnesium injections in patients with myofascial pain syndrome. <b>Objective:</b> This study aimed to compare the effectiveness of dry needling (DN) versus magnesium (Mg) infiltration in treating myofascial pain syndrome (MPS) with respect to pain reduction, quality of life (QOL), and symptoms of anxiety and depression.<b>Methods:</b> A randomized controlled trial (RCT) was conducted involving 45 patients with MPS. Participants were randomly assigned to Group A (dry needling) or Group B (magnesium infiltration). Both interventions were applied, and outcomes were assessed at baseline, one month, three months, and six months. Primary outcomes included pain intensity measured by the Visual Analog Scale (VAS), while secondary outcomes encompassed mental and physical health assessed via the SF-12 Mental and Physical Health Scales, and depression and anxiety levels measured by the Hospital Anxiety and Depression Scale (HAD).<b>Results:</b> Both interventions led to significant improvements in pain intensity, mental health, physical health, and reductions in depression and anxiety at one and three months. However, by six months, the differences in outcomes between the two groups were no longer significant. The mean changes in VAS scores, SF-12 scores, and HAD scores showed no significant differences between Group A and Group B at any follow-up time point.<b>Conclusion:</b> Dry needling and magnesium infiltration were both effective in the short to medium term for reducing pain and improving quality of life in patients with MPS. Despite significant improvements within each group, no superior effectiveness was observed between the two treatments. Both methods offer viable options for managing MPS, though additional long-term studies are needed to determine their sustained effectiveness and optimal use in clinical practice.<br>
