Preterm birth is one of the most pressing medical and social problems in obstetric practice and is directly associated with increased rates of perinatal morbidity and mortality. Recent scientific studies have demonstrated that metabolic disorders, particularly insulin resistance, as well as changes in the composition of intestinal and vaginal microbiota, play a significant pathogenetic role in the development of preterm birth. Microbiota imbalance against the background of insulin resistance may lead to activation of inflammatory processes, impairment of immune response, and dysfunction of the placental-fetal system.
The aim of this study is to evaluate the possibilities of predicting the development of preterm birth associated with microbiota changes based on insulin resistance. During the research, a prognostic model aimed at early identification of pregnant women belonging to a high-risk group is planned to be developed using clinical-anamnestic, biochemical, microbiological, and instrumental examination methods. The obtained results may contribute to the prevention of preterm birth, improvement of individualized preventive approaches, and expansion of early diagnostic opportunities in obstetric practice.
