Purpose: Posterior capsular rupture (PCR) is a significant complication that can profoundly impact patients, surgeons, and other stakeholders within the healthcare system during cataract surgery. This case study aims to introduce a new clinical sign, the "Ballerina" sign, which could potentially indicate posterior capsular rupture (PCR) during cataract surgery. Methods: In this case report, during a cataract operation of a 70-year-old man, a spiraling fragment of the nucleus was observed attached to a vitreous string. Results: During the procedure, an anterior vitrectomy was performed with no further intraoperative complications and a 3-piece lens was inserted into the sulcus. Positive visual outcomes were observed during follow-up appointments on the first day, two weeks, and three months after the operation. Conclusions: The “Ballerina” sign documentation in the medical literature is novel, and we anticipate that it will provide ophthalmic surgeons with valuable insight during cataract surgeries for early detection of PCR.
