Perfil ecoepidemiológico y clínico del escorpionismo en el estado Monagas, Venezuela (2002-2006)

RESUMEN Se evaluó el comportamiento del accidente por escorpiones en el estado Monagas, Venezuela, para el período 2002-2006. Se contabilizaron 984 accidentes, en promedio 197 casos por año. La frecuencia fue significativamente mayor en las estaciones de alta pluviosidad (378; 38,4%) y de baja pluviosidad (318; 32,35%) respecto a la de mediana pluviosidad (p < 0,001). Se registraron accidentes en 84,6% de los municipios (11/13) que conforman el estado. Los eventos se ubicaron fundamentalmente en dos secciones geográficas: a) el área norte del estado en la Serranía del Turimiquire [municipios Punceres (266; 27,0%), Acosta (129; 13,1%), Caripe (101; 10,3%), Piar (84; 8,5%), Bolívar (68; 6,9%) y Cedeño (48; 4,9%)] sumando 696 accidentes (70,7%) en una superficie de 5.259 km2, equivalente al 18,2% del total de la superficie del estado y b) el área de la altiplanicie o mesas (llanos altos) comprendiendo el municipio Maturín (249; 25,3%) con una superficie de 13.042 km2; que corresponde al 45,1% del total de la superficie de la entidad federal. Similarmente, los eventos graves (n = 56) se verificaron en ocho municipios (8/13; 61,5%); situados en la porción norte de la entidad; concentrando el 78,6% de los pacientes graves (44/56). La clínica sistémica fue significativamente autonómica de tipo colinérgica (432; 43,9%, p < 0,0002); con menor proporción la autonómica adrenérgica (39; 4,0%), cardiovascular (38; 3,9%), del sistema nervioso central (31; 3,2%), respiratoria (23; 2,3%) y motora (19; 1,9%). La mediana de incidencia para Monagas fue de 30,69 casos por 100.000 habitantes; con un mapa de endemicidad variable, clasificando los municipios (según los percentiles 90, 75, 50 y 25) en (1) municipios de muy Alta endemicidad (hiperendémicos), (2) alta endemicidad, (3) mediana endemicidad y (4) muy baja endemicidad; sin registrar decesos por escorpionismo en el periodo. Punceres (de muy alta endemicidad: hiperendémico) y Acosta (alta endemicidad) ostentaron, respectivamente, las medianas de incidencia más altas con 252,61 y 124,69 casos por cada 100.000 habitantes. Este trabajo actualiza y ratifica la importancia del escorpionismo en el estado Monagas como problema de salud colectiva, en términos de morbilidad, evidenciando la magnitud, trascendencia y gravedad de esta patología tropical.   PALABRAS CLAVE: Accidentes por escorpiones, incidencia, Tityus, Buthidae, Scorpiones.   ABSTRACT The behavior of scorpion sting-associated injuries in the state of Monagas, Venezuela, was evaluated for the period 2002-2006. A total of 984 injuries were recorded, averaging 197 cases per year. The frequency was significantly higher during periods of high rainfall (378; 38.4%) and low rainfall (318; 32.35%) compared to periods of moderate rainfall (p < 0.001). Injuries were reported in 84.6% of the municipalities (11/13) that make up the state. Events were primarily located in two geographic areas: a) the northern area of the state, within the Turimiquire range [municipalities of Punceres (266; 27.0%), Acosta (129; 13.1%), Caripe (101; 10.3%), Piar (84; 8.5%), Bolívar (68; 6.9%), and Cedeño (48; 4.9%)], totaling 696 accidents (70.7%) in an area of 5,259 km2, equivalent to 18.2% of the total area of the state, and b) the “mesas” (high plains) area comprising the municipality of Maturín (249; 25.3%) with an area of 13,042 km2; which corresponds to 45.1% of the total area of the state. Similarly, severe events (n = 56) occurred in eight municipalities (8/13; 61.5%); located in the northern portion of the state; concentrating 78.6% o severe patients (44/56). Systemic manifestations were significantly autonomic, of cholinergic type (432; 43.9%, p < 0.0002); with a lower proportion of adrenergic (39; 4.0%), cardiovascular (38; 3.9%), central nervous system (31; 3.2%), respiratory (23; 2.3%), and motor (19; 1.9%) manifestations. The median incidence for Monagas was 30.69 cases per 100,000 inhabitants; with a variable endemicity map, classifying municipalities (according to the 90th, 75th, 50th, and 25th percentiles) into (1) municipalities of very high endemicity (hyperendemic), (2) high endemicity, (3) moderate endemicity, and (4) very low endemicity, with no deaths recorded during the period. Punceres (of very high endemicity: hyperendemic) and Acosta (high endemicity) had, respectively, the highest incidence medians with 252.61 and 124.69 cases per 100,000 inhabitants. This study updates and confirms the significance of scorpionism in the state of Monagas as a public health problem, in terms of morbidity, highlighting the magnitude, significance, and seriousness of this tropical pathology.  KEY WORDS: Scorpion sting-associated injuries, incidence, Tityus, Buthidae, Scorpiones.
