VPRS 14407 Admission and Separation/Discharge Reports

The series comprise reports of patients admitted to and discharged from hospital. The initial report is usually patients admitted to hospital the previous 24 hours to midnight arranged alphabetically. The data in this report forms the basis for generation of the admission report. Information in the reports include medical/universal record number, name, sex, age, admission date, time, discharge/separation date, time, destination, diagnosis but this field is not always completed and account number. The print-outs are identified as 'Registers' but they are reports that are generated from the hospital information management accounting system.Birth Registers from 1930 to the present (not extant due to a flood that destroyed registers from 1942 - 1959), and Operation Registers from 1955 to the present are currently located with the hospital.
