DEVELOPMENT AND CLINICAL IMPLEMENTATION OF A COMPREHENSIVE MANAGEMENT ALGORITHM FOR CHOLANGIOSEPSIS

Cholangiosepsis is a life-threatening complication of biliary tract infections characterized by rapid progression to systemic inflammatory response and multiple organ dysfunction. Despite advances in surgery and intensive care medicine, mortality remains high due to delayed diagnosis, inadequate source control, and lack of standardized management strategies. The aim of this study was to develop and clinically evaluate a comprehensive management algorithm for cholangiosepsis integrating diagnostic assessment, risk stratification, intensive care, and surgical intervention. A retrospective–prospective analysis of patients with cholangiosepsis treated in a specialized hepatobiliary center was performed. The proposed algorithm emphasized early diagnosis, prognostic stratification, timely biliary decompression, and close coordination between intensive care and surgical teams. Implementation of the algorithm resulted in improved hemodynamic stabilization, reduced incidence of septic shock and multiple organ dysfunction, lower postoperative complication rates, and decreased mortality. The findings support the clinical effectiveness of a structured, multidisciplinary algorithm and highlight its potential role in standardizing cholangiosepsis management.
