This dataset contains qualitative interview material generated as part of a study exploring clinical decision‑making around admissions, referrals, and ongoing care management for people living with multiple long‑term conditions. The data consist of anonymised interview excerpts organised into a structured analytic framework, including themes, subthemes, and associated codes. The thematic structure reflects the complexity of decision‑making across clinical, organisational, and relational dimensions. The final analytic framework comprises four overarching themes: A journey of uncertainty, which captures how clinicians navigate ambiguity, fluctuating presentations, and incomplete information when making decisions about care; Within and beyond limitations, which describes how clinicians work within personal, professional, and systemic constraints and the strategies they use to adapt or overcome them; Structures of care, which focuses on organisational processes, service configurations, and system‑level factors that shape decision‑making and influence patient pathways; and Implementing relational care, which highlights the interpersonal, collaborative, and values‑based aspects of care, including communication, trust, and shared decision‑making. Each theme is supported by subthemes and detailed codes, with illustrative interview quotes that demonstrate how clinicians conceptualise and enact decision‑making in real‑world practice. The dataset provides insight into the interplay between clinical judgement, service pressures, and relational work in the management of people with multiple long‑term conditions.
