Post-Traumatic Stress Disorder (PTSD) and the neurobiological impact of trauma are among the most complex areas of psychiatric neuroscience, yet the field remains hindered by significant terminological fragmentation and conceptual overlap between "stress," "trauma," and "resilience." Researchers frequently conflate the normal physiological response to threat with the maladaptive, chronic reorganization of neural circuits that constitutes PTSD. This document presents the SERNA Conceptual Standard for the Neurobiology of Trauma and PTSD (SCS50), a unified theoretical framework designed to standardize definitions and model the causal pathways of traumatic memory and affective dysregulation. SCS50 establishes 12 canonical terms (SCS50-T01 through T12) that strictly differentiate between fear conditioning, inhibitory failure, and dissociative states. The standard introduces the Cortico-Limbic Dysregulation (CLD) model, which incorporates 10 testable hypotheses (SCS50-H01 through H10) mapping the bi-directional relationships between the amygdala, the medial prefrontal cortex, and the hippocampus. By resolving 8 documented field inconsistencies—such as the role of the hippocampus as a cause or consequence of trauma—SCS50 provides a rigorous foundation for cumulative science in trauma research, facilitating the development of targeted, mechanism-based therapeutic interventions.
