These are irregularity investigation files from the office of the General Manager, Safeworking (part of the Operations Division from 1974-1996). They consist of joint inquiries, joint investigations, jointly signed reports, comprehensive investigative reports and correspondence relating to train and wagon derailments/collisions, level crossing collisions, shunting mishaps, serious and fatal injuries to people. Included in some files are envelopes containing photographs that may cause distress to readers.The files are arranged in an alpha-numeric system. An A3 prefix refers to metropolitan irregularities, while an A4 prefix refers to country irregularities and an A6 prefix refers to injuries. The alpha-numeric prefix is followed by a year indicator, and then a sequential number. For example, A3.88.11241 is the file number for the irregularity No. 11241 documented during 1988, occurring in a metropolitan area. Registers and indexes are held.Files transferred to the State Archives relate to fatalities and major injuries. Series start and end dates are circa only. Custody History From 1955 until 2000 the State Rail Authority of New South Wales maintained an 'in house archives' consisting of the archival records of the Authority, its predecessor agencies and the private railway companies taken over by the government. The date ranges of the records were from 1849 until 2000. When the decision was made to close the 'State Rail Archives' the parts of the collection that were required as State archives were transferred to the State Records Authority of NSW. Those record series were incorporated into the State Records series system of archives control and were re-numbered. This series was formerly part of the State Rail Archives collection where it was identified as series R315. It may have been viewed in the State Rail Archives as series R315 and could have been previously cited as such in notes by researchers at State Rail Archives or in publications or unpublished works.
