Hepatic arterial infusion chemotherapy in patients with unresectble hepatocellular carcinoma: 3-year survival update

Hepatic arterial infusion chemotherapy (HAIC) with oxaliplatin plus raltitrexed showed a promising response rate in patients with unresectable hepatocellular carcinoma (HCC) in a phase 2 trial. Here, we report the updated 3-year survival data after enrollment. In this prospective trial, we enrolled patients with intermediate and advanced unresectable HCC. The treatment was HAIC with raltitrexed plus oxaliplatin. The objective responses were achieved in 19 (48.7%) of 39 patients in the intention-to-treat population. The median overall survival and progress-free survival were 11.2 and 6.5 months, respectively. Hepatocellular carcinoma (HCC) is a leading cause of cancer – related deaths in China, with a large proportion of unresectable cases at diagnosis. Existing treatments like transcatheter arterial chemoembolization (TACE) and systemic therapies have limitations, and the optimal hepatic arterial infusion chemotherapy (HAIC) regimen is yet to be determined.Phase 2, single – arm, single – center, prospective clinical study on HAIC with oxaliplatin and raltitrexed for intermediate- and advanced-stage unresectable HCC.In the intention-to – treat population, 48.7% (19/39) achieved objective response, and 79.5% achieved disease control. In the per-protocol population, 54.3% achieved objective response and 88.6% achieved disease control.Median overall survival was 11.2 months, with 1-year, 2-year, and 3-year survival rates of 46.1, 30.7, and 30.7% respectively.Compared to systemic therapies like sorafenib, HAIC with oxaliplatin and raltitrexed showed a higher objective response rate, suggesting potential superiority in treating advanced HCC.
