This dataset was created with the motivation to train Medical Large Language Models for improved fluency in clinical coding. It was extracted from the Centers for Medicare & Medicaid Services International Classification of Diseases, Tenth Revision, Clinical Modification 2025 files, covering patient encounters from October 1, 2024, to September 30, 2025. The dataset includes code description instructions, code chapter instructions, and synonymy instructions, provided in JSON format, demonstrating how to query the description, chapter, and synonyms of specific medical codes.
