Unveiling the Enigmatic Reality of Sexual Violence: A Case Study of a Teenage Girl with Multiple Infections and Pregnancy, and Its Pervasiveness in Society at Ndola Teaching Hospital.
Chibamba Moses, Kutemwa Namuthali (MS), Nalucha Muhau (MS), Seta Siziya PhD, Butemwe Kawanda (MS).
INTRODUCTION
Sexual violence is a pervasive issue that affects millions of people worldwide, particularly women and girls. According to the World Health Organization, 1 in 3 women experience sexual or physical violence in their lifetime, with many cases going unreported due to fear, shame, or cultural norms. This case presentation highlights the complex and devastating consequences of sexual violence, including multiple infections and pregnancy, and the urgent need for awareness, education, and support for victims. As the common phrase goes, "Silence is not always golden," it is crucial to break the silence and seek medical help immediately.
CASE PRESENTATION
We present the case of a 15-year-old female who was brought to our hospital three months after being raped by her boyfriend. Her family initially did not report the case due to fear of the consequences. Still, they finally decided to seek medical help when the girl started experiencing abdominal pain and vaginal bleeding. The girl reported that she had been in a sexual relationship with her boyfriend for several months before the rape occurred. She had not been using any form of contraception or protection during their sexual encounters.
Upon examination she was febrile, with a blood pressure of 100/67 mm Hg, a heart rate of beats per minute, laboratory tests showed positive results for a gravida test, indicating pregnancy. The girl also tested positive for HIV, syphilis, gonorrhea, and hepatitis B.

DECISION-MAKING
She was immediately started on appropriate treatment for these infections.
The family was counseled about the importance of reporting sexual violence and seeking medical help immediately. They were also informed about the legal procedures involved in reporting such cases and the support available to victims of sexual violence.
The girl underwent further counseling and support from our team of psychologists and social workers. She was provided with appropriate medical care and follow-up, and her family was encouraged to provide her with emotional and physical support.
DISCUSSION
Sexual violence is a complex issue with serious physical, emotional, and psychological consequences for survivors. The presented case of a 15-year-old female who suffered sexual violence resulting in multiple infections and pregnancy underscores the need for prompt detection and medical care. According to the World Health Organization (WHO), an alarming one-third of women globally have experienced physical and/or sexual violence at some point in their lives (WHO, 2021). This highlights the urgent need for awareness, education, and support for victims of sexual violence.
The case also emphasizes the importance of early detection and appropriate treatment of sexually transmitted infections (STIs) to avoid long-term complications such as infertility and chronic pelvic pain. According to a study by the Centers for Disease Control and Prevention (CDC), untreated STIs can lead to severe health consequences, including chronic pelvic pain, ectopic pregnancy, and infertility (CDC, 2021).

RECOMMENDATIONS
Healthcare providers should be trained to identify and report cases of sexual violence promptly. The WHO recommends that healthcare providers should provide comprehensive care to survivors of sexual violence, including medical care, psychosocial support, and legal assistance (WHO, 2021).
Education and awareness campaigns should be promoted to reduce the stigma surrounding sexual violence and encourage early reporting and medical attention. Governments and NGOs should provide adequate resources and support services to survivors of sexual violence. The WHO recommends that governments should prioritize funding for interventions to prevent and respond to sexual violence (WHO, 2021).

CONCLUSION
In conclusion, sexual violence is a significant public health issue that requires urgent attention and action from healthcare providers, policymakers, and the community. With increased awareness, education, and support services, we can empower survivors of sexual violence and reduce the prevalence of this pervasive issue.
